Morphology of wounds of the lower extremities in patients using platelet-rich plasma іnjections

The aim of the work: to investigate the main morphological signs of wound defects of the lower extremities of various etiologies during the first surgical treatment of the wound, as well as to compare the classical technique and application of wound debridement with platelet-rich plasma injections.
Materials and Methods. The results of histological material from wound defects of the lower extremities of various genesis in 71 patients were analyzed. The control group included 34 patients, the main group consisted of 37 people.
To assess the course of the wound process, a histological method of wound tissue research was used, with an emphasis on the quality of neoangiogenesis and the formation of connective tissue.
Results and Discussion. According to the results, endothelial cells with proliferative changes, expansion of capillaries in the granulation tissue, which is located at the bottom of the wound defect with vascular etiology, were found. Necrosis was a permanent feature of all preparations taken during the first surgical treatment of the wound. In most cases, fibrin was noted in the surface layer of the wound, as well as tissue and cellular detritus, which were impregnated with neutrophilic polymorphonuclear leukocytes and erythrocytes of a disturbed structure.
In the control group on the 14th day, the same type of changes were observed: the granulation tissue had a focal location, was distinguished by the vertical orientation of vessels, edema, and abundant leukocyte infiltration. The density of vessels was (41.0±10.8) units, and the area of vessels was (201±36.7) μm2.
On the 14th day, in the main group, bright granulations with marked marginal epithelization, around moderate swelling were noted. A better maturation of the granulation tissue was observed, the density of vessels is (45.3±9.3) units, the area of vessels is (246.75±77.20) μm2, which is 9.5% and 18.3% more, respectively, than the control group.
