MODERN APPROACHES TO THE TREATMENT OF GOUT AND STUDY OF THE RANGE OF MEDICINES FOR THE TREATMENT OF INFLAMMATORY DISEASES OF THE MUSCULOSKELETAL SYSTEM ON THE MODERN PHARMACEUTICAL MARKET OF UKRAINE

The aim of the work. To analyze the data of the scientific literature on modern approaches to the treatment of gout, to investigate the range of medicines used for the prevention and treatment of inflammatory diseases of the musculoskeletal system, to establish the role of herbal preparations in the treatment of these diseases and to determine promising active pharmaceutical ingredients for the development of new medicines in this direction actions.
Materials and Methods. Data from the State Register of Medicinal Products of Ukraine and the ATC classification system of the electronic resource Compendium.online were used to study the assortment of medicines used for the prevention and treatment of inflammatory diseases of the musculoskeletal system. Methods of information search and literature data analysis were used.
Results and Discussion. Current approaches to the treatment of gout include: lifestyle modification to control risk factors for cardiovascular complications, accompanied by hyperuricemia; treatment of an acute attack of gouty arthritis with the use of nonsteroidal anti-inflammatory drugs, colchicine, glucocorticoids, as well as therapy aimed at reducing the level of uric acid and prevention of new attacks of acute gout. However, the prevention of systemic lesions of internal organs deserves more and more attention, to which insufficient attention is paid.
A study of the assortment of medicines used for the treatment of gout, registered in the State Register of Medicines of Ukraine, was carried out. It was established that 31 medicines inhibiting the formation of uric acid were registered for the treatment of gout, including 9 medicines based on allopurinol and 22 medicines based on febuxostat, and 2 medicines that do not affect the metabolism of uric acid, based on colchicine. All medicines of this group are produced in the form of tablets. An analysis of the assortment of all medicines that affect the musculoskeletal system showed the presence of only a small number of herbal preparations in this group of medicines, most of which are semi-solid preparations for topical use containing essential oils. Medicinal products of plant origin that affect the musculoskeletal system are also represented by liquid medicines for topical application and only three preparations for oral use in the form of tablets and capsules, which contain an Harpagophytum procumbens extract, avocado and soybean oil, and Colchicum autumnale extract. Dietary supplements with an anti-inflammatory effect on the joints in the form of herbal teas are also available on the pharmaceutical market of Ukraine.
Conclusions. Taking into account modern approaches to the treatment of gout, medicinal products based on plant raw materials, which have anti-inflammatory and analgesic effects, contribute to the reduction of pain and swelling, reduce the level of uric acid and have a hepatoprotective effect, deserve great attention. Promising for the creation of new medicines for the prevention and treatment of gout are plant extracts obtained from medicinal plant raw materials, in particular Silybi mariani fructs, Gentianae roots, Sаmbuci flowers, Betulae and Cynara scolymus leaves.
