PREVALENCE AND CLINICAL MANIFESTATIONS OF CARDIOVASCULAR SYSTEM LESIONS IN LYME BORRELIOSIS

SUMMARY. The article provides analyzis of the scientific literature data on the epidemiology and pathogenesis of heart lesions in Lyme borreliosis (LB); the frequency, timing and clinical significance of its cardiological symptoms. The reasons for clinical diagnostics difficulties in Lyme carditis at the current stage are considered as well. It has been proven that in general the relevance of Lyme borreliosis continues to grow in a number of countries, despite the success in treatment, which is due to climatic changes and other factors.
Clinical manifestations of Lyme carditis are diverse, those that occur in the period of early dissemination of the pathogen are described in more detail, in particular, such a potentially life-threatening condition as atrioventricular block. At the same time, there are some reports of no less dangerous heart disorders, such as sick sinus syndrome, sinoatrial block etc, which are usually not in the focus of investigations. A number of LB clinical aspects today require a more in-depth study, in particular, regarding myocarditis and pericarditis of Borrelia etiology, which are often asymptomatic. The least studied are cardiac complications in the chronic stage of LB, about which there are isolated reports in the scientific literature.
Thus, the issue of diagnosis of myocarditis and late cardiac lesions in LB requires further research and development of clear diagnostic criteria. In endemic areas, vigilance is needed regarding the possibility of heart lesions of Borrelia etiology, not only in the case of AV-block, but also in other rhythm and conduction disorders.
