Сomparison of efficiency of laparoscopic fundoplication according to nissen in patients with expressend extraesophageal laryngo-pharingeal manifestations of gastroesophageal reflux disease

Abstract

Gastroesophageal reflux disease (GERD) with extraesophageal manifestations, which present in 10 % of patients, is serious clinical problem. Surgical procedure is necessary if conservative treatment is not effective. The article represents results of surgical treatment of 251 patients with extraesophageal GERD manifestations operated from 2001 till 2011. Nissen fundoplication was made for 221 (88 %) patients, Toupet fundoplication was made for 30 (12 %) patients. Mean postoperative DeMeester score in patients with Nissen fundoplication was significantly lower than in patients with Toupet fundoplication, 11.6±4.8 and 23.8±4.2 (p<0.05), respectively. Mean postoperative index of symptom was significantly lower in patients who underwent Nissen fundoplication, compared to patients who underwent Toupet fundoplication, 10.4±3.4 and 17.8±4.6 (p<0.05), respectively. In conclusion, laparoscopic fundoplication is effective for both classical form of GERD and GERD with extraesophageal manifestations. Nissen fundoplication is better for patients with extraesophageal manifestations of GERD.
