The aim of the work: Based on a specific clinical case of combined endovesical lithotripsy (mechanical and holmium laser) for a large calculus in an orthotopic ileal neobladder, to draw attention of urologic surgeons to the peculiarities of neobladder lithiasis, intraoperative risks, and tactical considerations that ensure safety and radicality of the intervention. The combined use of a mechanical cystolithotriptor and holmium laser represents an effective and justified alternative to more invasive methods of stone removal from intestinal urinary reservoirs, and the presented clinical case demonstrates the possibility of preventing complications and achieving an optimal surgical outcome.
