MULTIDISCIPLINARY APPROACH IN THE SURGICAL TREATMENT OF PATIENTS WITH RADICULAR CYST

The aim of the work: to optimize treatment approaches for patients with radicular cysts based on our clinical observations.
Materials and Methods. Under the supervision of the Medical Center “LORIMED” were 12 patients with such pathology. We present a clinical case of one of them. Patient T. was 57 years old. A month ago, there was a history of purulent discharge from the nose with an unpleasant smell and severe pain in the area of the affected tooth and right maxillary sinus. Symptomatic treatment with antibiotic therapy was previously performed. The condition temporarily improved, after which she was referred for surgical treatment, which was performed under general anesthesia.
Results. After a joint examination with the dentist, a decision was made to perform an endoscopic right-sided maxillofacial surgery, submucosal resection of the nasal septum, and extraction of the affected tooth. After a month – the antrostomy is wide and functional, the sinus mucosa is clean, there are no complaints. The mucous membrane of the gum in the area of the extracted tooth has no signs of inflammation, no pathological changes. Implantation with osteoplasty is planned in the future. The main conditions for the successful treatment of patients with this pathology are a multidisciplinary approach with the involvement of a dentist and that is an extraction of the causative tooth and the formation of a wide functional antrostoma.
Conclusions. Endoscopic maxillotomy allows to perform complete sanation of the affected sinus with minimal traumatization to surrounding tissues, absence of cosmetic defects, and allows simultaneously correct intranasal structures for better sinus ventilation.
