QUALITY OF LIFE OF PATIENTS BEING ON KIDNEY REPLACEMENT THERAPY TAKING INTO ACCOUNT THE PRESENCE OF TYPE 2 DIABETES MELLITUS

SUMMARY. The number of patients with end-stage renal disease requiring hemodialysis is growing steadily. The lives of patients with end-stage renal disease are reorganized and adapted to the changes that occur with the start of dialysis therapy. The intensity of mental and physical condition of the patient significantly affects the level of quality of life.
The aim – to determine the level of quality of life in patients undergoing renal replacement therapy, taking into account type 2 diabetes.
Material and Methods. The study was conducted at the Center for Nephrology and Dialysis of KE "M. V. Sklifosovsky Poltava Regional Clinical Hospital of Poltava Regional Council”. The study involved 93 patients, of which 44 patients on programmed hemodialysis and 49 who were in the control group - patients with stage I–II CKD. In turn, the experimental group (patients of dialysis therapy) was divided into two subgroups: 20 patients (ЕG1) with diabetic nephropathy (DN), 24 – with other diseases (ЕG2). The average duration of treatment with renal replacement therapy was 6.2±6.7 years. The control group was also divided into two subgroups: 26 patients with DN (KG1), and 23 – without DN (KG2). Quality of life was determined by a quality of life questionnaire (SF-36). The statistically significant difference between the comparison groups was determined using the Student's parametric criterion.
Results. There is a statistically significant difference between ЕG2 and KG2 p<0.001, ЕG1 and KG1 p≥0.05 when determining the scale of physical functioning. The scale of pain intensity characterizes in ЕG1 and KG1 a statistically significant difference p<0.01, in ЕG2 and KG2 p<0.005. The overall health scale characterizes a statistically significant difference in both ЕG1 and KG1 and ЕG2 and KG2, p≤0.005 and p<0.001. The vital activity scale shows a statistically significant difference between ЕG1 and КG1 p<0.001 and ЕG2 and KG2 p< 0.01. The scale of social functioning characterizes a statistically significant difference, respectively in ЕG1 and KG1 p<0.05, and in ЕG2 and KG2 p<0.01. There is a statistically significant difference between both ЕG1 with KG1 and ЕG2 with KG2, p=0.01 and p<0.01, respectively, when determining the scale of mental health.
Conclusions. The difference between the indicators of the psychological component in dialysis patients, regardless of the presence of diabetes, with control patients is very high. Thus, the average indicators of the psychological component of dialysis groups are 49.7±11.5 44.5±10.6, respectively, while in the control groups 91.4±2.1 and 90.1±8.2, respectively. There is a statistically significant difference p<0,001 in both psychological and physical components of quality of life assessment.
