IMPACT OF TIME OF HOSPITALIZATION OF MYOCARDIAL INFARCTION PATIENTS ON EFFECTIVENESS OF MEDICAL CARE

Keeping health is a priority of modern society. One of the complex medical and social problems in Ukraine are cardiovascular diseases. In recent years there is a trend towards continual increase in their prevalence and morbidity.
The aim of the study – to analyze the impact of the time of hospitalization on treatment effectiveness in myocardial infarction (MI) patients who underwent coronary angiography and coronary artery stenting (CAS) and balloon angioplasty (BAP).
Materials and Methods. The study used data from medical records of 173 MI patients who were hospitalized in Ternopil Municipal City Hospital No 2. Methods used: copying of data, medical-statistical, analytical.
Results and Discussion. The study revealed the impact of time of hospitalization on the organization of medical care and treatment efficacy in patients with MI. Vast majority of patients with MI (86.1 %) are delivered for hospital treatment within 12 hours of the onset. In the first 2 hours, patients are often delivered by the first aid crews. In case of a more extended time from the onset, patients often called on their family doctor, or came to the Resuscitation and Intensive Care Unit/Cardiology Department on their own or accompanied by relatives (co-workers). There is a tendency of increased share of cases of referral of patients for coronary angiography, CAS and BAP by other medical facilities with the increase of time from the onset. We have found that patients with myocardial infarction without ST-segment elevation often arrived more than 24 hours after the onset. We established a tendency of increase in average duration of treatment in the Resuscitation and Intensive Care Unit of patients hospitalized with increasing time before hospitalization. There is also an increase in time from first contact with a hospital doctor until coronary angiography in patients with late hospitalization.
Conclusions. The data indicate a relationship between time of hospitalization and organization of medical care and treatment efficacy for MI patients. In late hospitalization, significant role is played by personal factor of patients and their relatives.
