ASSESSMENT OF COGNITIVE DYSFUNCTION AND HEALTH-RELATED QUALITY OF LIFE IN PERSONS WITH DIABETES MELLITUS WHO HAD ISCHEMIC STROKE AT THE LEVEL OF PRIMARY HEALTH CARE

Assessing health-related quality of life (HRQOL) after an ischemic stroke provides not only information about the physical and psychological aspects of the patient’s health, but also allows doctors and nurses to better assess the recovery process after a stroke and is essential for increasing the effectiveness of treatment.
The aim of the study – to evaluate level of cognitive dysfunction and indicators of HRQOL in patients with diabetes mellitus who had an ischemic stroke 1 year ago.
The main part. Монреальського когнітивного тесту (MoCA). The study involved 14 patients diagnosed with type 2 diabetes mellitus (DM2) who had an ischemic stroke 1 year ago and 12 patients who had an ischemic stroke 1 year ago without type 2 diabetes mellitus. To assess HRQOL, the short version of the Health Questionnaire – 36 (SF 36) was used. Cognitive impairment was determined using the Montreal Cognitive Assessment (MoCA). Comparing the parameters of HRQOL in persons without DM2 who had an ischemic stroke 1 year ago vs control group, a significant decrease in all parameters of HRQOL was established. The greatest changes were observed in physical functioning, pain scale, general health and integrative parameter. Moreover, in persons with DM2 who had an ischemic stroke 1 year ago, most parameters of HRQOL were significantly lower compared to persons without DM2. Analyzing the distribution of study participants regarding the severity of cognitive impairment according to the results of the MoCA test, we found that in the group without DM2, 25.0 % of individuals had mild cognitive impairment, 58.3 % of individuals had moderate cognitive impairment, and only 16.7 % of individuals – severe cognitive impairment; in the group with DM2, no individuals were identified who had mild cognitive impairment, 64.3 % of individuals had moderate cognitive impairment and 35.7 % of individuals had severe cognitive impairment.
Conclusions. The aggravating effect of comorbid DM2 on the HRQOL and cognitive functioning of persons who had an ischemic stroke 1 year after the incident has been established, which must be taken into account when implementing rehabilitation programs after a stroke.
