CLINICAL CASE OF RUMINATION SYNDROME AND AEROPHAGY IN A BOY OF PRIMARY SCHOOL AGE

The aim of the study – to analyze modern ideas about the etiology and treatment bases of such a functional rational gastrointestinal disorder, as a rumination syndrome, diagnosed in children and adolescents.
This article examines modern views on functional disorders of the gastrointestinal tract in children. The frequency of functional gastrointestinal disorders is quite high in all age groups, but in older children functional diseases are often on the verge of pathology, which leads to increased interest of pediatricians in their etiology, pathogenesis and therapy. Although these disorders do not pose a significant threat to life and, as a rule, do not have a significant impact on children's health in the future, they affect the quality of life, lead to a large number of unjustified hospitalizations, the use of unwarranted pharmacotherapy, disrupt the psychological climate of the family. Rumination syndrome (RS) is a functional gastrointestinal disorder that is increasingly diagnosed in older children and adolescents, but the epidemiology of this condition in school-age children has not been sufficiently studied. The pathophysiology of RS involves an increase in intragastric pressure, which occurs due to unintentional contraction of the abdominal wall muscles and in the presence of low pressure in the lower esophageal sphincter, which causes retrograde movement of gastric contents into the esophagus. To date, controlled studies of the treatment of rumination syndrome are insufficient. The basis of the treatment of rumination syndrome is explanatory and behavioral therapy, which consists of methods of eliminating habits that compete with the urge to vomit. Aerophagia is a functional gastrointestinal disorder characterized by repetitive air swallowing, abdominal distension, belching and flatulence. Early recognition and diagnosis of functional aerophagia is required to avoid unnecessary, expensive diagnostic investigations or serious clinical complications. The article presents a clinical case of rumination and aerophagia syndrome in a school-age boy who has not been correctly diagnosed and treated for a long time, has caused concern among parents and significantly worsened the child's quality of life.
