СOMPLEX IMPROVEMENT OF TREATMENT IN PREGNANT WOMEN WITH PANCREATIC EXTERNAL-SECRETORY DYSFUNCTION

The aim of the study – the improvement of treatment the clinical manifestations of early gestosis in the first trimester of pregnancy by correction of the functional liver damage and external-secretory pancreatic function.
Materials and Methods. Forty-five pregnant women were examined with manifestations of moderate severity early gestosis.
Among them, 21 pregnant women were in comparator group and received treatment in accordance with the recommendations for the management of pregnant women with early gestosis manifestations. On the other hand, 24 patients of the second group in addition with main treatment had a prescription drug, which contains dry extract of ginger 150mg in combination with 6 mg Vitamin B6 1 capsule three times per day during 14 days.
Results and Discussion. In the examined pregnant women, there are significant violations of biochemical parameters that describe the functional state of the liver and pancreas in the form of cholestatic, cytological and mesangiogenic-inflammatory syndromes, manifested by a significant increase in the content of total bilirubin in blood serum due to an increase in the fraction of direct bilirubin, an increase in the activity of serum aminotransferases ALAT and ASAT, an indicator of thymol, alkaline phosphatase.
Low levels of fecal pancreatic elastase-1 and high ketosis suggest external-secretory dysfunction of the pancreas. The use of a dry extract of ginger in a dose of 150 mg in combination with vitamin B6 at a dose of 6 mg three times a day in the main group for 3–4 days from the beginning of treatment allowed not only to eliminate the marked clinical symptoms of early gestosis, but also to restore the basic laboratory biochemical markers of the liver exchange. Simultaneous increase of fecal pancreatic elastase-1 and absence of ketosis is evidence of stabilization of the functional state of pancreas.
Conclusions. Functional liver disorders and external-secretory function of the pancreas as markers of disease in pregnant women with moderate severity early gestosis were revealed. For the pregnant with manifestations of early gestosis in the first trimester it is recommended to include in the complex therapy of the disease a dry extract of ginger in a dose of 150 mg in combination with vitamin B6 at a dose of 6 mg three times per day for at least 14 day. This medications promote the normalization of clinical manifestations (improvement of well-being, elimination nausea, reduction of episodes of vomiting) and laboratory markers from 3–4 days of treatment in the examined patients.
