LIFE QUALITY OF PATIENTS WITH CHRONIC PANCREATITIS IN COMBINATION WITH VIRAL HEPATITIS C

Abstract

The article analyzes the effect of concomitant viral hepatitis C on the quality of life of patients with chronic pancreatitis. The conducted studies showed a negative impact of concomitant pathology, which was manifested by significantly more pathological changes compared with the control group.

 Material and Methods. A total of 57 patients with chronic pancreatitis with concomitant viral hepatitis C were examined. The comparison group consisted of 20 persons with chronic pancreatitis. To establish a clinical diagnosis of chronic pancreatitis and viral hepatitis C, all patients were examined in accordance with the protocols adopted for patients on orders No. 638 of the Ministry of Health of Ukraine dated 09.10. 2014 and No. 729 of the Ministry of Health of Ukraine dated July 18, 2016. The diagnosis of chronic pancreatitis and viral hepatitis C was verified on the basis of anamnesis data, clinical manifestations and research findings. The research was conducted in patients with chronic pancreatitis in the phase of exacerbation and unstable remission, patients with viral hepatitis C – in the phase of stable remission. The clinic and health assessment was conducted using the SF-36 general questionnaire. To assess the severity of gastroenterological symptoms and quality of life, the Russian version of the GSRS (Gastrointestinal Symptom Rating Scale) was used.

Results. The average quality of life of the questionnaire SF-36 on the scale of physical functioning was in the group of patients with CP with concomitant HCV (51.6±4.8) points and (72.3±5.1) marks in the control group; on the scale of the role of physical functioning: (30.4±4.3) and (47.6±5.1) points; on the scale of pain: (42.8±5.2) and (71.5±4.7) points; according to the general health scale: (33.6±2.1) and (48.3±2.4) points. The average quality of life of the questionnaire SF-36 on the scale of viability was (26.1±4.6) points in the group of patients with CP with concomitant HCV, and in the group of patients with CP – (52.3±5.4) points; on the scale of social functioning: (45.8±4.9) (75.4±5.3) points; on the scale of role-playing emotional functioning: (34.3±4.8) and (63.7±5.1) bali; on the scale of mental health: (44.3±4.5) and (55.4±4.1) points. The life quality of patients with chronic pancreatitis with concomitant viral hepatitis C in comparison with the group of patients with chronic pancreatitis on the scales of physical health was reduced by 20.3 points, according to the scales of psychological health – by 24 points. Analyzing the data of the GSRS questionnaire for patients with chronic pancreatitis with concomitant viral hepatitis C, it was found that the leading place in the clinic of the disease was pain, reflux, and dyspeptic syndromes (p>0.05). These syndromes should be considered as the main factor in reducing the quality of life in patients with chronic pancreatitis with concomitant viral hepatitis C. The increase in indices on the scale of constipation and diarrhea in this group was less significant.

Conclusions. According to the data of the study a lower quality of life in patients with concomitant viral hepatitis C was established.
