PATHOLOGICAL RECURRENT PNEUMOTHORAX AS A SYMPTOM OF BULLOUS LUNG DISEASE: THE PRACTICAL CASE

Spontaneous pneumothorax (SP) is an independent disease or complication in patients with the lung bullous disease, manifested by the accumulation of air in the pleural cavity due to the occurrence of pathological communication of the pleural cavity with airways. Epidemiology of SP in Ukraine, as well as in the world, is presented by limited data, so in the UK the prevalence of SP is 16.8 per 100 thousand per year (24 – for men and 9.8 – for women); the frequency of primary SP among men in the U.S., is 7.4 per 100 thousand, and in Sweden – 18–28 per 100 thousand. An actual issue of clinical practice is the question of compliance with the verification algorithm of the cause of spontaneous pneumothorax, in particular, as the first manifestation of pulmonary disease and the need for differential diagnosis of the causes of SP. The article describes the peculiarities of diagnostics of the lung bullous disease in the dynamics of symptomatology progression within 3 years from the moment of the first clinical symptoms, the primary manifestation of which was SP, by the example of the clinical case of the patient 30 years without accompanying pathology with the experience of smoking for 13 years (1 pack daily). The analysis of features of diagnostic tactics of these patients at the outpatient stage is presented, typical diagnostic errors in the evaluation of complaints and diagnostics of the lung bullous disease, the first clinical manifestation of which was SP, are analyzed. The issue of importance of a family doctor in accompanying and monitoring patients with spontaneous pneumothorax is emphasized.
