METHODOLOGICAL AND CLINICAL ASPECTS OF MANAGING HEMATOLOGICAL PATHOLOGY PATIENT IN A FAMILY DOCTOR PRACTICE ON A CLINICAL CASE EXAMPLE

SUMMARY. The development of family medicine requires a comprehensive approach to examining the patient and providing modern medical care to patients. In this regard, the requirements for training a family doctor at both undergraduate and postgraduate levels are constantly increasing. Blood diseases, in particular, oncological ones, constitute a major medical and social problem, therefore improving their early diagnosis is great importance.
The aim – to improve the training of general practitioners in identifying the earliest signs of oncohematological pathology by improving the methodology and clinical analysis in the educational process of specialists using the example of analyzing a clinical case from personal practice.
Material and Methods. bibliosemantic, observation and analysis methods were used.
Results. A survey of family doctors showed their insufficient understanding of their competencies in helping patients with hematological pathology, in particular, oncohematological, when early diagnosis is of great importance, as well as further management of these patients regarding comorbid conditions. Therefore, explaining and reminding family doctors of competencies in their training should take their due place, because this category of patients usually directs their first visit to a family doctor. Due to the imperfect ability to palpate the lymph nodes and spleen, family doctors often do not apply this in practice, so practicing skills and working "at the patient's bedside" is urgently needed. The clinical example given in the article and its analysis is an effective method of training doctors, in particular with oncohematological pathology.
Conclusions. Family doctors often underestimate their importance in providing medical care to oncohematological patients due to the lack of theoretical knowledge and practical skills, so improving the training of family doctors should compensate for these shortcomings. In the presence of non-specific syndromes – intoxication, inflammatory and infectious, as well as more specific syndromes – anemic and hemorrhagic, oncohematological diseases should always be suspected. Integral components of the examination of a patient with polymorbid pathology should be a blood test, palpation of the spleen and lymph nodes. Increased attention of doctors during teaching and detailed clinical analysis of clinical cases on the topic should be integral components of effective training of general practitioners.
