EFFICIENCY OF EXOGENOUS MELATONIN USE IN TUBAL INFERTILITY

Tubal-peritoneal infertility is a common factor in female infertility, accompanied by a disruption in the pro/antioxidant system. Melatonin is a powerful neutralizer of free radicals, so the use of exogenous melatonin, as an antioxidant therapy for such infertility, is quite reasonable.
The aim of the study – to determine the effectiveness of antioxidant therapy with exogenous melatonin in infertility of tubal origin.
Materials and Methods. The study involved 65 women with tubal-peritoneal infertility who were treated using assisted reproductive technology. Depending on the desire of the woman to receive different treatment, the patients were divided into 2 groups. The group 1 included 33 patients who received exogenous melatonin, and the group 2 included 32 women who were treated according to the standard therapy. The control group consisted of 35 practically healthy fertile women with realized reproductive function. The level of endogenous melatonin, oxidative protein modification, the content of TBA-active products, the activity of enzymes superoxide dismutase (SOD) and catalase, the level of fragmented DNA (f-DNA) was determined for all women.
Results and Discussion. The determination of endogenous melatonin in serum showed a statistically lower level of hormone in women with infertility, compared with control group. After receiving melatonin for three months, the level of lipid peroxidation and oxidative modification of proteins decreased, while the activity of antioxidants in SOD and catalase increased both in the endometrium and in the follicular fluid. Correlation analysis between the indicators of the antioxidant system, the level of f-DNA in the endometrium and the follicular fluid in tubal-peritoneal infertility showed a significant effect of exogenous melatonin on the normalization of the free radical process, which confirms its positive role in the regulation of oxidative stress (directly or indirectly) and apoptosis.
Conclusions. Antioxidant therapy with exogenous melatonin for three months is enough for the effective activation of endogenous antioxidant defense mechanisms in the body, which leads to a decrease in the content of free radicals and the normalization of apoptosis in the reproductive tract of women.
