CLINICAL-LABORATORY, FUNCTIONAL AND PSYCHOLOGICAL CHANGES AMONG PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE IN CONNECTION WITH POST-TUBERCULOSIS CHRONIC LUNG DISODERS

In the conditions of pulmonary tuberculosis (TB) epidemic and the high incidence the number of people with post-TB chronic lung disorders (PTBLDs) increases. However, there are no studies that would examine the peculiarities of COPD in patients with PTBLDs and the relationship between residual lung damage and the development of pulmonary hypertension in the literature available to us. In a study the impact of PTBLDs on the prevalence of COPD was assessed.
The aim of the study – to learn the clinical course, laboratory and functional changes among patients with COPD in association with PTBLDs.
Materials and Methods. A total of 127 patients with GOLD grade 2, 3, age (56.6±1.2) years, were examined, among whom 47 patients with age (58.7±1.7) years were diagnosed with PTBLDs. All patients underwent general clinical examination, Acid-Fast Bacillus (AFB) Testing, spirometry, echocardiography, Spielberg anxiety scale, Beck depression scale, and The Short Form (36) Health Survey.
Results and Discussion. It was noted that 37.01 % of the surveyed were diagnosed with PTBLDs. According to clinical trials and spirography among patients with PTBLDs, more severe symptoms of COPD was observed. The relationship between the presence of PTBLDs and the development of pulmonary hypertension and iron deficiency states was revealed. Also, among patients with PTBLDs, adverse psychological changes were revealed, as evidenced by the higher rates of depression and trait anxiety compared with patients without lung damage. Quality of life indicators were lower among patients with PTBLDs compared to patients without them. Women with PTBLDs were found to be less likely than men.
Conclusions. Thus, among patients with COPD and PTBLDs co-morbidity, more severe symptoms, elevated levels of depression and trait anxiety and low quality of life were observed. There was also a link between the presence of PTBLDs and the development of pulmonary hypertension and iron deficiency states. Among women, combined pathology was less common than among men.
