MORPHOLOGICAL FEATURES OF POSTOPERATIVE SCARS OF SKIN IN THE APPLICATION OF COMBINED PREVENTION (EXPERIMENTAL STUDY)

Abstract

The problem of pathological postoperative cicatrization of skin remains relevant and insufficiently studied. Scar is a product of skin regeneration by connective tissue structures after its damage or inflammatory process for supporting integrity of body's covers.

The aim of the study – to learn the effect of extracorporeal shock-wave therapy (ESWT ) and Strataderm silicone gel on the morphological features of reparative regeneration of scar tissue of the skin in experimental animals.

Materials and Methods. Experimental study was performed on 16 male rabbits divided into four groups and learnt in them the effect of complex prophylaxis of pathological post-operative skin scars formation.

Results and Discussion. The skin glands were arranged in one row superficially and hair follicles were completely absent. The scar was formed by coarse disordered collagen fibers. Collagen fibers were thin and there was a very small amount of thin muscle fibers between them. Cutaneous appendages were better developed: the glands were arranged in one row superficially being larger than in the control group. Sebaceous glands were grouped in two or three rows. Hair follicles regenerated and they were arranged in several rows. The scar was formed by disordered collagen fibers. A multirowed stratum corneum was revealed in the morphological study of animals of experimental group number. Completely restored appendages of the skin were observed: sebaceous glands and hair follicles regenerated and formed groups arranged in several rows. The scar was formed by coarse ordered collagen fibers.

Conclusions. Experimental study proves the effectiveness of the combined prevention of pathological postoperative cicatrization. Extracorporeal shock-wave therapy was established to affect the regeneration of elastic and collagen fibers and smooth muscle cells of the dermis positively. In its turn, the use of a silicone gel contributes to the formation of multinucleated epidermis and the complete restoration of the architectonics of cutaneous appendages. Combined use of ESWT and Strataderm silicone gel integrates the merits of both methods and promotes the formation of normotrophic scar, which involves the balance of such processes as the formation of the epidermis and the regeneration of collagen and elastic fibers along with appendages of the skin.
