ASSESSMENT OF COMPLIANCE WITH CURRENT STANDARDS FOR THE DIAGNOSIS OF SEXUAL DYSFUNCTION IN MEN

Abstract

Purpose: to assess the completeness of diagnostic examinations of men with sexual dysfunctions in accordance with the requirements of the clinical guidelines of the European Association of Urologists (2025). Materials and Methods. The data from the medical records of 402 men with sexual dysfunctions–patients of three privately owned medical facilities in Ivano-Frankivsk–were analyzed. Results. Most often (but not in 100 % of cases), the diagnosis was made on the basis of an ultrasound examination of the prostate gland and scrotal organs (79.3–83.1 %), as well as the determination of testosterone levels in the blood (70.0–79.7 %, with premature ejaculation – only 42.7 %). The use of instrumental examinations such as duplex ultrasound examination of the arteries of the penis in erectile dysfunction (covered by only 66.8 %) and biotensiometry in premature ejaculation (41.6 %) and orgasmic dysfunction (40.0 %) is insufficient. The examinations necessary to assess possible metabolic disorders in erectile dysfunction (31.0-54.3 % of patients) and determine hormonal status are not performed sufficiently. Thus, the determination of prolactin levels, recommended for all sexual disorders except premature ejaculation, was performed in 44.3-55.0 % of patients, luteinizing hormone in erectile dysfunction – 22.0 %, thyroid hormones in hypoactive sexual desire disorder – 11.5 %, and orgasmic dysfunction – 10.0 %. It has been established that doctors do not make sufficient use of easy-to-use and inexpensive questionnaires standardized for the diagnosis of sexual dysfunction (23.6-53.3 %) and do not assess the psychological status of such patients at all. Conclusions. The scope of examinations conducted in healthcare facilities for the diagnosis of male sexual dysfunction does not comply with the guidelines recommended by the European Association of Urologists (2025).
