DIAGNOSTIC ALGORITHM AND THE VOLUME OF STUDIES OF PATIENTS WITH PLEURAL TB EMPYEMA

Abstract

Purpose: to improve the diagnostic algorithm and scope of investigations of patients with pleural tuberculosis empyema and to establish the factors that promote minimally invasive thoracic surgery (VTS, VATS) treatment.

Materials and Methods. Modern diagnostical methods of pleural TB empyema and thoracoscopy methods of verification of pleural tuberculosis, TB empyema, surgical methods of verification of tuberculosis and methods of minimally invasive surgical treatment are presented.  

Results.  The diagnostic algorithm has been developed and implemented into health care practice for the early diagnostics of pleural TB empyema. The cytological and molecular genetic methods of verification of  pleural TB empyema, methods for special examination of patients with pleural empyema and biopsy methods under ultrasound, pleurography, contrast CT have been improved and implemented for thoracocentesis and videothoroscopy in patients at different stages of  treatment polyclinic and hospital institutions.

Conclusions. Pleural biopsy with bacteriological and cytological researches, minimally invasive videothoroscopy with biopsy of pleural tubercules, hystopathological study of caseous ulcers, necrosis, and suspicious of tuberculosis pleuro-pulmonary fistula are of great importance for the diagnosis of pleural TB empyema in preclinical phase at the present stage.
