EFFECTIVENESS OF PRP THERAPY IN PATHOGENETIC CORRECTION OF ENDOTOXICOSIS AND IMMUNOMETABOLIC DISORDERS IN ACUTE PERITONITIS

SUMMARY. Given the pronounced endogenous intoxication and oxidative stress in acute peritonitis, the search for pathogenetic correction agents is relevant. The use of platelet-rich plasma (PRP) is promising due to its ability to modulate the inflammatory response and stabilize cellular homeostasis.
The aim – to establish the effectiveness of PRP in correcting endotoxicosis, oxidative stress, and immune reactivity indicators under conditions of experimental acute peritonitis (AP).
Material and Methods. The study was conducted on male white rats with an AP model (injection of 10 % fecal suspension). Animals of the main group were injected with PRP after 24 hours. In the dynamics (1, 4, 7 and 10 days) the indicators of endotoxicosis, oxidative stress, antioxidant protection, immune response and liver condition were determined. Statistical processing of the results was carried out using the Student's t-test.
Results. Induction of AP was accompanied by the rapid development of endotoxicosis: after 24 hours the erythrocyte intoxication index (EII) increased to (94.80±1.32) % (p<0,001). By the 10th day, the level of MSM₂₅₄ increased by 1.6 times, and the CIC content reached a peak – 189.50±1.67 standard units. The development of pathology was characterized by a peak of cytolysis on the 7th day and an intensification of oxidative stress (MDA – (5.45±0.08) μmol/kg) against the background of depletion of antioxidant protection. The use of PRP provided a reliable correction of disorders: on the 10th day, EII decreased to (63.79±1.14) %, and the CIC level normalized to (66.17±1.11) standard units (p1<0,001). Restoration of catalase activity, reduced glutathione level and normalization of ALT index ((55.50±2.17) U/l) were noted, which confirms the hepatoprotective and antioxidant effects of PRP due to the action of biologically active platelet factors.
Conclusions. PRP therapy is an effective means of pathogenetic correction in acute peritonitis, as it significantly reduces the manifestations of endogenous intoxication, eliminates oxidative stress and normalizes the body's immune reactivity. The obtained data justify the feasibility of including PRP in the complex therapy of inflammatory processes of the abdominal cavity for the prevention of organ dysfunctions and stimulation of regeneration.
