Objective. To assess the prevalence of behavioral NCD risk factors among adolescents in Lviv region in 2025 and differences by age, sex, residence, and IDP status. Materials and Methods. Anonymous survey of 702 students (grades 6–11; 11–17 years; mean 13.7) from seven schools. Descriptive statistics; 95 % CIs by Wald for means and Wilson for rare proportions, χ² and t-tests for subgroup comparisons (JASP). Indices: behavioral domain of Life’s Essential 8, Composite Healthy Lifestyle Score, KIDMED, simplified Healthy Eating Index (HEI; ІЗХ). Results. Behavioral risk factors for NCDs are established during adolescence and shape adult health; the wartime context may alter exposure and behaviors. Ever-smoking 20.4 % (17.4–23.8 %); current smoking 6.6 % (4.7–8.4 %). Ever alcohol 43.0 % (39.4–46.6 %); regular (≥weekly) 0.4 % (0.0–0.9 %). Perceived harms of vaping: 54.2 % (50.3–58.0 %) “as harmful,” 39.2 % (35.4–43.0 %) “more harmful,” 16.1 % (13.2–18.9 %) “less harmful,” 1.1 % (0.3–1.9 %) “harmless.” Physical activity ≥5 sessions/week: 26.1 % (22.8–29.3 %); 3–4 sessions: 43.7 % (40.0–47.4 %); 1–2 sessions or less: 30.2 % (26.8–33.6 %). Weekly sleep disturbances: 32.8 % (29.4–36.4 %); chronic fatigue: 43.0 % (39.4–46.6 %); sleep duration not reported: 1.5 % (0.6–2.4 %). Negative HEI in 12.7 % (10.3–15.2 %); average composite healthylifestyle score: 2/5. Conclusions. A cumulative risk profile is evident–low physical activity, mixed/unhealthy diet, sleep problems, and early contact with psychoactive substances. Interdisciplinary school- and community-based interventions should jointly address activity, nutrition, tobacco/alcohol prevention, stress regulation, and risk awareness.
