A MULTIDISCIPLINARY APPROACH TO THE OBSERVATION OF A PATIENT WITH CHRONIC HEART FAILURE AND COMORBID PATHOLOGY – EMPHASIS ON SELF-MANAGEMENT

Purpose: to determine the effectiveness of a multidisciplinary approach to the observation of a patient with CHF and comorbid pathology, assessing the dynamics of clinical, hemodynamic parameters, indicators of tolerance to physical exertion, quality of life, compliance with treatment in the process of modern personalized therapy and teaching patients the principles of self-management/self-care.
Materials and Methods. 53 patients with clinically prominent signs of heart failure and comorbid pathology were examined. A multidisciplinary approach to dynamic monitoring of patients was used. Contact with patients was made directly, as well as by phone, online, via Viber, WhatsApp, Telegram messengers, the duration of observation was 6 months. We studied the dynamics of general clinical indicators, functional status of patients, compliance with treatment and quality of life of patients.
Results. The use of a multidisciplinary approach to the dynamic monitoring of patients with CHF and comorbid pathology increased the effectiveness of treatment due to their acquisition of new knowledge about their disease, confidence in the possibility of effective self-management/self-examination, which was confirmed by the results of the assessment of the dynamics of general clinical indicators, functional status, treatment compliance and quality of life.
Conclusions. Better results in increasing treatment compliance and quality of life indicators of patients with chronic heart failure were observed after using a comprehensive approach: modern drug treatment and patient education of self-management elements at home with the active involvement of a multidisciplinary team and the use of telephone communication.
