JUSTIFICATION OF THE CHOICE OF THE METHOD OF ORAL SANATION IN YOUNG CHILDREN

Abstract

Purpose: to establish a safe period of time for various dental procedures in children under the age of 3 years on an outpatient dental appointment; determine the indications for the rehabilitation of the oral cavity under general anesthesia.

Materials and Methods. Dental examination and dental procedures of varying severity were performed for 110 children under the age of 3 years (432 teeth were treated) at the Dental Medical Center at the National Medical University named after O. O. Bohomolets.

Children were divided into two groups depending on the choice of treatment method: group I – children, sanation of the oral cavity which was carried out in conditions of general anesthesia (38 children); group II – consisted of children whose oral sanation was performed without general anesthesia (72 children).

In order to determine the oxygen saturation of the brain, the cerebral oximetry method was used. To predict cognitive changes against the background of organic brain disorders, we used the results of J. Meixensberger et. al. 1998, where the main task of the dentist was considered to prevent a decrease in cerebral oximetry by more than 20 %.

Results. Indications for oral rehabilitation in conditions of general anesthesia:

- superficial caries, regardless of its course at dm ≥6;

- middle caries, regardless of its course at dm ≥2;

- deep caries, chronic course with dm ≥2;

- deep caries, acute course with dm ≥1;

- chronic fibrous pulpitis with dm ≥1;

- chronic periodontitis.

Conclusions. The choice of the method of oral cavity rehabilitation in children under the age of 3 years does not depend on the type of temperament and type of phobia.
