PRINCIPLES FOR THE USE OF BRONCHODILATORS IN THE DIAGNOSTICS OF BRONCHIAL OBSTRUCTIVE SYNDROME

Abstract

Progressive rapid increase in number of obstructive pulmonary diseases is a concern for doctors around the world and requires quality diagnostics. Spirometry is the most common and informative technique used to assess pulmonary function. The bronchial obstruction reversibility testing with bronchodilators is an integral part of it.

The aim of the study – to present the modern approaches to the use of bronchodilator medications in the diagnosis of bronchial obstructive syndrome.

Results. In particular, an updated current recommendations for test conditions in the bronchial obstruction reversibility, list of indications and contraindications to the main groups of bronchodilators, their dosage and route of administration, use of delivery devices for aerosol drug (it depends on the age and condition of the patient), modern approach to withholding times for bronchodilators before bronchodilator responsiveness testing, new spirometry quality assessment system are presented. Particular attention is paid to the mechanisms of action of bronchodilators on muscle tone of the airways, to the conditions, which can have affect the severity of this effect, and, accordingly, to the quality of the results analysis.

Conclusions. Compliance with the modern recommendations on the methodology for performing the test, the choice of bronchodilators, on their dosage and route of administration, on the algorithm for the correct analysis of the data should ensure bronchial obstruction reversibility test execution and interpretation of its results at the appropriate modern level.
