МОРФОЛОГІЧНІ ЗМІНИ ЕНДОМЕТРІЯ У ПАЦІЄНТОК ІЗ ХРОНІЧНИМ ЕНДОМЕТРИТОМ ПІСЛЯ PRP-ТЕРАПІЇ

Анотація

Мета дослідження – оцінити динаміку морфологічних змін ендометрія у пацієнток із хронічним ендометритом до та після застосування PRP-терапії.

Матеріали та методи. У проспективному дослідженні взяли участь 200 жінок із безпліддям (середній вік (32,5±1,2) року), які проходили лікування методами допоміжних репродуктивних технологій. Усім пацієнткам проводили офісну гістероскопію з біопсією ендометрія у середній проліферативній фазі менструального циклу. Морфологічну оцінку здійснювали за стандартним гістологічним фарбуванням гематоксиліном та еозином. Хронічний ендометрит було діагностовано у 93 пацієнток. Після проведення PRP-терапії виконували повторну гістероскопію з біопсією. Оцінку змін здійснювали за напівкількісною шкалою (0–3) за такими критеріями: набряк, структура залоз, клітинність строми, запальна інфільтрація, судинні зміни та інші морфологічні ознаки.

Результати дослідження та їх обговорення. Дослідження підтвердило провідну роль морфологічної діагностики ендометрія як ключового методу верифікації хронічного ендометриту та об’єктивного моніторингу ефективності лікування, що дозволяє оцінити не лише наявність запалення, а й його стадію, активність і глибину структурних змін; найбільш інформативними маркерами визначено лімфоцитарну інфільтрацію, стан строми, ступінь фібротичних і судинних змін та синхронність дозрівання ендометрія. Встановлено, що антибактеріальна терапія забезпечує лише частковий регрес запалення без повноцінного відновлення архітектоніки тканини, тоді як додавання PRP-терапії суттєво покращує регенерацію, мікроциркуляцію та структурну організацію ендометрія, знижуючи вираження патологічних змін майже вдвічі.

Висновки. Найбільшого вираженого відновлювального ефекту досягають при застосуванні комбінованого підходу зі включенням противірусної терапії, що забезпечує майже повну нормалізацію морфофункціонального стану ендометрія, підкреслюючи важливість комплексного впливу на всі ланки патогенезу та його значення для підвищення ефективності подальших репродуктивних втручань.
