ПОСТМЕНОПАУЗАЛЬНА ОСТЕОСАРКОПЕНІЯ

Анотація

Мета дослідження – аналіз даних літератури та результатів власних досліджень із проблеми остеосаркопенії в постменопаузі: патофізіологічні механізми розвитку, фактори ризику, підходи до діагностики, лікування, прогнозування ризику переломів і їх профілактики. Постменопаузальна остеосаркопенія, розвиток якої зумовлений генетичними факторами, зміною складу тіла, низькою фізичною активністю, дефіцитом естрогенів і вітаміну D, призводить до зниження м᾽язової маси, сили, функціональних можливостей, а також до зниження мінеральної щільності кісткової тканини, що реалізується в збільшенні частоти переломів, інвалідизації та смертності. У даний час відсутнє специфічне лікування саркопенії, первинною є профілактика. Рання діагностика остеосаркопенії, прогнозування ризику переломів та їх профілактика – шлях до збереження якості та незалежності життя жінки.

Матеріали та методи. Обстежено 65 жінок у віці (55,7±0,6) року, в постменопаузі з остеопорозом. Оцінку стану кістково-м᾽язової системи та ризик падінь вивчали за допомогою функціональних тестів, ультразвукового дослідження основних УЗ-параметрів m. quadriceps femoris на апараті «Toshiba aplio 300».

Результати дослідження та їх обговорення. Функціональні тести: порушення координації, стійкості і FraxAll (R=-0,67), порушення координації, стійкості, ризик падінь і FraxAll (R=0,64), динамометрія і FraxAll (R=-0,77), динамометрія і Fraxhit (R=-0,70). УЗ-параметрів м᾽язів: ехогенність м᾽яза і ширина м᾽яза (R=0,78), ехогенність м᾽яза і кут перистості (R=0,99).

Висновки. Остеосаркопенія часто спостерігається у жінок у постменопаузі, призводить до зниження м᾽язової маси, сили, функціональних можливостей, а також до зниження мінеральної щільності кісткової тканини, що погіршує якість життя, сприяє збільшенню частоти падінь і, відповідно, ризику переломів. Функціональні тести стану скелетної тканини мають високу інформативність у плані прогнозування ризику переломів. Ультразвукове вимірювання параметрів м᾽язів є інструментом для ранньої діагностики і моніторингу лікування саркопенії.
