Abstract

SUMMARY. Chronic heart failure (CHF) is one of the most common and severe progressive conditions of the cardiovascular system. Despite improvements in diagnostics and increased public awareness, it remains a significant medical and social challenge today.

The aim – to provide a clinical evaluation of biochemical and echocardiographic indicators of endotoxicosis in patients with CHF of varying severity.

Material and Methods. The study involved 39 patients with Stage I–IIB CHF and 10 practically healthy individuals. Clinical, laboratory (MSM 280, TNFα, MDA, CIC, SOD), and echocardiographic (EchoCG) examinations were performed. A correlation analysis was conducted to examine the relationships between clinical, laboratory, and echocardiographic parameters.

Results. Significant correlations were established between EchoCG parameters (left ventricular end-systolic dimension (LVESD), ejection fraction (EF)) and clinical signs of hemodynamic decompensation. Associations were identified between the structural and functional parameters of the heart and markers of endotoxicosis, specifically between LVESD and MSM 280, as well as MDA. Correlations with TNFα and certain indicators of antioxidant defense mostly did not reach statistical significance.

Conclusions. The progression of CHF is accompanied by an increase in the manifestations of endotoxicosis and structural-functional changes in the heart. Significant correlations were established between EchoCG indicators and clinical signs of hemodynamic decompensation. Changes in left ventricular dimensions and its functional parameters are associated with an increase in endotoxicosis markers, indicating their role in myocardial remodeling. Including markers of endotoxicosis in the comprehensive assessment of CHF is appropriate.
