СУЧАСНІ КОНЦЕПЦІЇ МУЛЬТИДИСЦИПЛІНАРНОГО МЕНЕДЖМЕНТУ РУБЦЕВИХ ДЕФОРМАЦІЙ ЩЕЛЕПНО-ЛИЦЕВОЇ ДІЛЯНКИ

У статті представлено системний аналіз етіології, патогенезу та сучасних методів корекції рубцевих деформацій обличчя та шиї. Особливу увагу приділено молекулярним механізмам формування гіпертрофічних та келоїдних рубців. Обґрунтовано необхідність застосування мультимодального підходу, що включає фармакотерапію, фізичні методи впливу (лазерні технології, кріодеструкція) та хірургічну реконструкцію. Висвітлено специфіку реабілітації пацієнтів із рубцевими контрактурами щелепно-лицевої ділянки в контексті відновлення функцій зубощелепної системи.
Мета дослідження – систематизувати та науково обґрунтувати сучасні мультидисциплінарні підходи до діагностики, профілактики та лікування рубцевих деформацій щелепно-лицевої ділянки з метою оптимізації естетичних та функціональних результатів реабілітації пацієнтів, зокрема з бойовою травмою.
Матеріали та методи. Проведено поглиблений бібліосемантичний та аналітичний огляд сучасної фахової літератури за останні 10 років із використанням наукометричних баз даних PubMed, Scopus, Web of Science та Google Scholar. Проаналізовано міжнародні клінічні рекомендації (International Advisory Panel on Scar Management) та результати рандомізованих клінічних досліджень щодо ефективності консервативних, ін’єкційних, апаратних та хірургічних методів корекції рубців.
Результати досліджень та їх обговорення. Встановлено, що етіопатогенез гіпертрофічних та келоїдних рубців базується на дисрегуляції фази проліферації, опосередкованій гіперактивністю трансформуючого фактора росту (TGF-β) та порушенням співвідношення колагену I/III типів. Визначено, що монотерапія рубцевих деформацій обличчя демонструє низьку клінічну ефективність. Обґрунтовано ступінчастий алгоритм терапії: застосування силіконових матеріалів та компресійної терапії для гідратації рогового шару та регуляції активності фібробластів; інтралезійне введення кортикостероїдів (тріамцинолон) та ферментних препаратів (колагеназа) для інгібування синтезу глікозаміногліканів; апаратні методи: диференційоване використання лазерних технологій на основі селективного фототермолізу (PDL 585–595 нм) для корекції васкуляризації та абляційних систем (Er:YAG 2940 нм, CO2 10 600 нм) для ремоделювання рельєфу шкіри (акцентовано увагу на перевагах ербієвого лазера для роботи в естетично значущих зонах обличчя завдяки високому коефіцієнту поглинання води; хірургічний етап – показаний при функціональних порушеннях (мікростомія, рубцеві контрактури, виворіт повік) із застосуванням методів місцевої пластики та ліпофілінгу.
Висновки. Ефективний менеджмент рубцевих деформацій щелепно-лицевої ділянки вимагає переходу від емпіричного лікування до патогенетично обґрунтованих комбінованих протоколів. Інтеграція зусиль стоматологів, щелепно-лицевих хірургів та комбустіологів є критично важливою для відновлення функцій зубощелепної системи (жування, артикуляції) та психосоціальної реінтеграції пацієнтів.
