Анотація

Гранулоцитарний анаплазмоз людини (ГАЛ) – гостра бактерійна трансмісивна хвороба з ураженням гранулоцитів, збудником якої є Anaplasma phagocytophilum. Це зооноз, що передається через укуси кліщів Ixodes ricinus. Найчастішими симптомами є гарячка, нездужання, міалгія, біль у голові, анорексія. У значної частини пацієнтів ГАЛ маніфестує в поєднанні з іншими кліщовими інфекціями. Полімеразна ланцюгова реакція (ПЛР) розглядається як основний специфічний метод для підтвердження діагнозу ГАЛ у гострому періоді захворювання. Хоча A. phagocytophilum чутлива до всіх тетрациклінів, у лікуванні препаратом вибору є доксициклін.

Потепління клімату є одним із ключових чинників, що сприяють розширенню ареалу іксодових кліщів і збільшенню тривалості їх сезонної активності. Найактивніші природні осередки інфекції поширені в лісових і лісостепових зонах. Все частіше виявляють заражених кліщів у міських парках. Важливу роль відіграють і фактори урбанізації та розростання міст, що призводить до більшого контакту міського населення з місцями проживання кліщів.

Комплексне розуміння профілактики ГАЛ та інших кліщових інфекцій органічно вписується у концептуальні рамки «Єдиного здоров’я» (One Health) й обґрунтовує складну взаємопов’язаність і взаємозалежність здоров’я людей, тварин і навколишнього середовища, а також необхідність усунення дисциплінарних і професійних бар’єрів у підходах до охорони здоров’я. Відповідно заходи профілактики, що охоплюють лише популяцію людей, є принципово недостатніми. Для реалізації підходу «Єдине здоров’я» необхідна координація зусиль фахівців із клінічної медицини, ветеринарії, санітарно-епідеміологічної служби та екології, а також формування інтегрованих систем епідеміологічного нагляду, що охоплюють популяції людей, тварин і навколишнє середовище. На поширення кліщів впливає рослинність, оскільки одні види сприяють росту і розмноженню їх, інші, навпаки, – пригнічують завдяки наявним компонентам з репелентними властивостями (полин лікувальний, гвоздика садова). Як доведено, корінь-трава, японський барбарис, жимолость створюють густі зарості із сприятливим мікрокліматом, що значно підвищує щільність популяції кліщів і тривалість їх життя. Стратегія екологічного управління включає також модифікацію рослинного покриву через усунення інвазивних чагарників і листопадового опаду, що обмежує виживання переносників.
