Формування біоплівок та їх значення в розвитку інфекційних процесів у людини

Біоплівки є складними структурованими мікробними спільнотами, які формуються на біологічних тканинах і поверхнях медичних пристроїв, а також відіграють важливу роль у розвитку хронічних інфекцій. Формування біоплівки відбувається за участю позаклітинного полімерного матриксу (ЕПС), до складу якого входять полісахариди, білки та позаклітинна ДНК (e-DNA). Цей матрикс забезпечує мікроорганізмам захист від імунної відповіді організму та значно підвищує їх стійкість до антимікробних препаратів. Найчастіше до утворення біоплівок залучені такі патогени, як Staphylococcus aureus, у тому числі метицилін-резистентні штами (S. aureus MRSA), Escherichia coli, Pseudomonas aeruginosa та Haemophilus influenzae.
Біоплівки відіграють важливу роль у патогенезі багатьох хронічних і рецидивних інфекцій. Вони беруть участь у розвитку остеомієліту, інфекційного ендокардиту, інфекцій сечових шляхів (ІСШ), а також катетер-асоційованих інфекцій. Особливе значення біоплівкові інфекції мають при захворюваннях дихальної системи, зокрема при хронічному обструктивному захворюванні легень (ХОЗЛ), муковісцидозі та бронхоектазах, де формування біоплівок сприяє персистенції патогенів, хронізації запального процесу та зниженню ефективності антибактерійної терапії.
Крім того, біоплівки можуть формуватися на поверхнях різноманітних медичних виробів, таких як сечові катетери, протези клапанів серця та інші інвазивні пристрої. Утворення біоплівок на цих поверхнях створює сприятливі умови для тривалого існування мікроорганізмів, їх розмноження в організмі хазяїна. Внаслідок цього значно ускладнюється лікування інфекцій, підвищується ризик рецидивів захворювання та зростає потреба у тривалій або комбінованій антибактерійній терапії.
