TREATMENT OF LUNG TUBERCULOSIS IN HOSPITAL VS OUTPATIENT – PATIENT’S OPINION

Abstract

The aim of the study – to evaluate the patient’s with lung tuberculosis attitude to the choice of in-hospital and outpatient treatment.

Materials and Methods. The questionary prepared by us was provided to 105 patients with lung tuberculosis. It was analyzed type of the patients and their attitude to the choice of in-hospital an outpatient treatment, frequency and reasons of chemotherapy interruption.

Results and Discussion. Around 73.3 % patients picked up the option of in-hospital treatment till the significant improvement and negative MBT test. The reason of such choice was unwilling to spread over the infection to the family members and nearest people (specifically such opinion was worded by 96.0 % patients of 4th category) and also better conditions and higher efficacy of in-hospital treatment. 8.6 % of patients supported the option of only outpatient department treatment. Treatment interruption in hospital in 79.2 % of cases was the result of it adverse effects, and in outpatient department in 65.0 % cases it was the result of patients ignorance of treatment and absence of treatment control. The analysis of responses based on residence, concomitant diseases, bad habits, chemoresistancy of MBT resulted in separation of specific attitude of these patients to treatment that might be taken into the account when the treatment approach will be selected.

Conclusions. We understand that our hospitals are on transition, but total rejection of in-hospital stage of treatment until significant improvement of patients and negative MBT test is not a good idea. The in-hospital stage of treatment is supported with epidemiological and social conditions, necessity of evaluation of tolerability of treatment and its correction and also difficulty of providing controlled treatment in outpatient departments especially in rural locations.
