ЗМІНИ ЛІПІДНОГО ОБМІНУ ТА РЕЗИСТИНУ У ПАЦІЄНТІВ ІЗ ПОЧАТКОВИМИ СТАДІЯМИ ХРОНІЧНОЇ ХВОРОБИ НИРОК, АСОЦІЙОВАНОЇ З ОЖИРІННЯМ

РЕЗЮМЕ. Існує тісний зв’язок між хронічною хворобою нирок та ожирінням. Ожиріння впливає на функцію нирок через механізми хронічного запалення, порушення регуляції адипокінів, активацію ренін-ангіотензин-альдостеронової системи, внутрішньониркового накопичення ліпідів.
Метою нашого дослідження була оцінка змін ліпідного профілю та динаміки адипокіну – резистину та їх взаємозв’язку у пацієнтів із ранніми стадіями хронічної хвороби нирок, асоційованої з ожирінням.
Матеріал і методи. Обстежено 300 хворих на ХХН. Хворих було поділено на 4 групи: І (70) – хворі з ХХН І ст. з нормальною масою тіла, ІІ (72) – хворі з ХХН ІІ ст. з нормальною масою тіла; ІІІ (70) – хворі з ХХН І ст. та ожирінням, ІV (88) – хворі з ХХН ІІ ст. та ожирінням. Контрольну групу склали 30 практично здорових осіб. Усім хворим визначали ІМТ, розраховували швидкість клубочкової фільтрації (ШКФ) за формулами CRD-EPI на основі рівня креатиніну, цистатину С та їх поєднання (CRD-EPIcysC/cr) (мл/хв/ 1,73 м2 ). Рівні цистатину С, резистину, СРБ, ФНП-α у сироватці крові визначали методами імуноферментного аналізу. Рівень загального холестерину, ліпопротеїдів низької та високої щільності визначали колориметричним методом.
Результати. Рівні резистину були вищими в групах хворих із ХХН та ожирінням. Вміст резистину зростав у випадках зниження ШКФ у пацієнтів ІІІ та ІV груп. У пацієнтів ІІІ та ІV груп був виявлений прямий середньої сили кореляційний зв’язок між резистином та показником ОТ/ОС. У пацієнтів з ХХН та ожирінням у міру зниження функції нирок спостерігалося зростання рівня тригліцеридів і ЛПНЩ, а також підвищення рівня прозапальних цитокінів, таких як ФНП-α та СРБ, особливо у хворих з ІІ стадією ХХН та ожирінням. Високі рівні ЛПНЩ, тригліцеридів корелюють як з високим рівнем резистину, так і з вищими показниками низькоінтенсивного запалення у пацієнтів ІІІ та ІV груп.
Висновки. У хворих на ХХН та ожиріння виявлені підвищені рівні резистину, які зворотно корелюють зі швидкістю клубочкової фільтрації. Також у пацієнтів з ХХН на тлі ожиріння в сироватці крові виявлено високі рівні ЛПНЩ, тригліцеридів, які корелюють як з високим рівнем резистину, так і з вищими показниками низькоінтенсивного запалення.
