ОСОБЛИВОСТІ КАЛЬЦИФІКАЦІЇ КЛАПАННОГО АПАРАТУ СЕРЦЯ У ПАЦІЄНТІВ, ЯКІ ПЕРЕБУВАЮТЬ НА НИРКОВО-ЗАМІСНІЙ ТЕРАПІЇ ПРОГРАМНИМ ГЕМОДІАЛІЗОМ

Анотація

РЕЗЮМЕ. Вивчення особливостей кальцифікації клапанного апарату серця як прояв ХХН-МКП, є вкрай актуальною проблемою в клінічній практиці у хворих з ХХН VД стадії, оскільки цим обумовлене підвищення кардіо-васкулярної захворюваності та смертності у вказвній категорії пацієнтів.

Мета – визначити фактори ризику ККА у пацієнтів із ХХН VД стадії шляхом аналізу ключових клініко-лабораторних показників.

Матеріал і методи. Проведено одноцентрове крос-секційне когортне дослідження на базі відділення гемодіалізу КНП “Тернопільська обласна клінічна лікарня” ТОР, в рамках якого обстежено 134 хворих з ХХН VД стадії. У всіх пацієнтів визначали рівень вітаміну D, С-реактивного білка (СРБ), ліпідний профіль, сечовину, креатинін, загальний білок та альбумін, феритин, ШОЕ, інтактний паратгормон (іПТГ), кальцій (Са), фосфор (Р), активність лужної фосфатази (аЛФ), рівень глікозильованого гемоглобіну (HbA1c). Усім хворим виконано ехокардіоскопію у двовимірному режимі та рентгенографію хребта та кінцівок. Рівень циркулюючого склеростину (SCL) визначали за допомогою автоматизованого сендвіч-ІФА ELISA SOST для людини з використанням технології SimpleStep (ab221836) компанії Abcam (Кембридж, Велика Британія).

Результати. ККА виявлено у 48,5 % обстежених, середній вік яких становить 59,32±11,7 роки; 34 (52,3 %) з яких чоловіки та 31 (47,7 %) жінки. За результатами дослідження, пацієнти з ККА достовірно відрізнялися за віком (59,32±11,17 проти 51,42±14,14; p=0,002); за кількістю пацієнтів, що хворіють цукровим діабетом – в групі з ККА таких пацієнтів достовірно більше (16/24,6 % проти 8/11,6 %; p=0,009); за тривалістю перебування на терапії гемодіалізом – пацієнти з ККА достовірно довше перебували на гемодіалізі (58,00 проти 29,00; p=0,003). У групі пацієнтів з ККА був достовірно вищий рівень сироваткового Р (1,99 проти 1,65; p=0,003), що поєднувався з нижчим рівнем Са (2,06 проти 2,26; p=0,027); достовірно вищий рівень циркулюючого SCL (760,04 проти 448,52; p=0,021); достовірно вищий рівень СРБ (12,08 проти 8,19; p=0,044). Крім того у пацієнтів з ККА виявлено достовірно вищу частоту низькоенергетичних переломів (вертебральних і невертебральних) (21/32,3 % проти 6/8,7 %; p=0,001).

Висновки. ККА серця, як прояв ХХН-МКП, є поширеним явищем у пацієнтів з ХХН VД стадії. Її розвиток головним чином пов’язаний з віком, тривалістю перебування на нирковозамісній терапії програмним гемодіалізом, цукровим діабетом. Лабораторним підгрунтям ККА є важкі порушення мінерального обміну у вигляді гіперфосфатемії, гіпокальціємії на тлі дефіциту вітаміну D в умовах хронічного запалення. Клінічно ККА асоціюється з підвищенням частоти вертебральних і невертебральних переломів у хворих з ХХН VД стадії, що є підтвердженням концепції кістково-судинної осі. Крім того ККА поєднується з достовірним підвищенням рівня циркулюючого склеростину, що є сучасним маркером ХХН-МКП у пацієнтів з ХХН VД стадії.
