ТРУДНОЩІ ДИФЕРЕНЦІЙНОЇ ДІАГНОСТИКИ СКЛЕРОДЕРМОПОДІБНОГО ПАРАНЕОПЛАСТИЧНОГО СИНДРОМУ Й СИСТЕМНОЇ СКЛЕРОДЕРМІЇ ЯК ФАКТОРА ОНКОЛОГІЧНОГО РИЗИКУ

Системна склеродермія (ССД) – автоімунне захворювання сполучної тканини, що характеризується прогресуючим фіброзом шкіри та внутрішніх органів. Характерні клінічні ознаки ССД можуть маскувати онкологічні захворювання, а паранеопластичний процес, у свою чергу, маскується симптомами ССД.
Мета дослідження – показати необхідність диференційної діагностики склеродермоподібного паранеопластичного синдрому та ССД як фактора онкологічного ризику.
Матеріали і методи. Проведено огляд та аналіз даних науково-медичної літератури.
Результати досліджень та їх обговорення. Імунні реакції, що беруть участь у патогенезі ССД, можуть сприяти розвитку онкологічної патології; тому пацієнти з факторами ризику розвитку захворювання вимагають спостереження за можливим розвитком неопластичного процесу. Пацієнти з ССД і аутоантитілами до РНК-полімерази III мають значний ризик розвитку раку. Ризик канцерогенезу найвищий у перший рік після постановки діагнозу ССД. З іншого боку, симптоми ССД можуть бути маскою різних видів раку. Паранеопластичний склеродермоподібний синдром, безпосередньо не пов’язаний з первинною пухлиною і її метастазами, а зумовлений складними вторинними запально-дегенеративними змінами системного, локального або дистантного характеру, часто перебігають у вигляді проявів “перехрещення” (overlap) ревматичного та пухлинного захворювань. Передбачається, що ракові клітини можуть індукувати синтез речовини, що ініціює фіброз шкіри і внутрішніх органів. Диференціація між ідіопатичною формою ССД, склеродермоподібним паранеопластичним синдромом і розвитком раку на тлі ССД спричиняє безліч труднощів. У статті представлено аналіз диференційно-діагностичних заходів, пропонується схема скринінгу для ведення пацієнтів із діагнозом ССД, які перебувають у групі ризику розвитку малігнізації.
Висновки. Результати аналізу джерел літератури доводять актуальність поглибленого вивчення пацієнта із вперше виявленими склеродермоподібним синдромом, особливо в похилому віці.
