UNIVERSITY CLINICS OR CONSOLIDATED CLINICAL BASES: ORGANIZATIONAL MODELS, ADVANTAGES AND PROSPECTS OF DEVELOPMENT

Purpose. The goal is to justify the prospective directions of development and optimization of the interaction of medical institutions and institutions of higher education in the conditions of the need to optimize the system of train- ing medical personnel. Materials and methods. In this study, we used a complex of general scientific and special research methods, in particular, we used methods of analysis and synthesis, a systematic approach, comparative methods, structural and functional analysis and content analysis of regulatory and legal documents. Results. It is shown that traditional approaches to the organization of clinical training of future specialists are gradually losing their effectiveness, which is due to the gap between the educational process and real clinical practice, limited resources and the inertia of the integration of science and practice in the educational process. Of particular importance in this context is the need to transform the clinical bases of medical institutions of higher edu- cation, which simultaneously perform educational, scientific and medical functions. In this connection, the question arises of choosing the optimal model for the organization of clinical bases – university clinics or consolidated clinical bases that will be able to ensure a balance between the quality of medical education, the effectiveness of medical care and economic feasibility. Conclusions. The problem of choosing an effective organizational model of clinical bases is the need to achieve an optimal balance between integration and flexibility, centralized management and autonomy, as well as between educational and clinical priorities. It is substantiated that a hybrid model combining a centralized core (university clinic) and a network of hospital bases is optimal.
