ЕПІГЕНЕТИЧНА ТА ХРОНОФАРМАКОЛОГІЧНА МОДУЛЯЦІЯ КЛІТИННОГО СТАРІННЯ ШКІРИ

Анотація

Вступ. Сучасна дерматокосметологія переходить від естетичної корекції зовнішніх ознак старіння до глибинного управління біологічним віком шкіри (Skin Longevity). Відомо, що мелатонін є ключовим регулятором циркадних ритмів, а порушення «біологічного годинника» клітин є першопричиною зниження регенераторного потенціалу шкіри. Лікарський засіб рапаміцин, через його унікальну здатність впливати на шлях mTORC1, дозволяє очищувати клітини від «біологічного сміття» (аутофагія) та боротися зі станом inflammaging (хронічним запаленням). Екзосоми є найбільш перспективним методом безклітинної терапії, що забезпечує точкову доставку генетичної інформації для «перепрограмування» старих клітин на омолодження. Мета дослідження. Проаналізувати роль циркадних осциляторів у механізмах старіння шкіри та обґрунтувати ефективність мультимодальних стратегій регенерації, що базуються на застосуванні екзосомальної терапії, хронобіотичної підтримки мелатоніном та метаболічної модуляції шляху mTORC1 рапаміцином. Методи дослідження. Проведено критиний аналіз та систематизацію наукових даних щодо молекулярних механізмів старіння шкіри та порівняльний аналіз результатів клінічних і експериментальних досліджень відповідно до мети дослідження. Пошук релевантних джерел здійснювався в електронних базах PubMed, Scopus та ClinicalTrials.gov за ключовими словами: skin longevity, circadian rhythms, exosomes, mTOR, melatonin. Результати та обговорення. Було встановлено, що ключовим механізмом старіння шкіри є десин- хроноз – порушення роботи внутрішніх «годинникових» генів (зокрема BMAL1 та PER2). Це призводить до того, що клітини втрачають здатність ефективно відновлюватися під час сну. Аналіз дії мелатоніну показав, що він виступає не просто антиоксидантом, а потужним хронобіотиком. Він проникає крізь гематоенцефалічний бар'єр та мембрани клітин, зв'язується зі специфічними рецепторами та відновлює циркадний ритм фібробластів. Це стимулює вироблення власного колагену та еластину, а також активує ферменти, що захищають ДНК від ультрафіолетового пошкодження. Пригнічення білкового комплексу mTORC1 за допомогою малих доз рапаміцину запускає в шкірі процес аутофагії. Це фактично «система утилізації» всередині клітини, яка переробляє пошкоджені білки та органели. Завдяки цьому зменшується кількість сенесцентних клітин («клітин-зомбі»), які провокують хронічне запалення та руйнують навколишній матрикс. Дослідження впливу екзосом продемонструвало їхню роль як інтелектуальної системи доставки. Екзосоми, отримані з мезенхімальних стовбурових клітин, містять унікальний набір мікроРНК та факторів росту. Потрапляючи в глибокі шари дерми, вони передають «сигнал молодості» старим фібробластам, пригнічуючи активність ферментів, що руйнують колаген, та ініціюючи бурхливий неоколагенез. Висновки. Інноваційна anti-age- стратегія має бути комплексною та спрямованою на відновлення внутрішніх біологічних ритмів шкіри. Мелатонін, рапаміцин та екзосоми є перспективними інноваційними засобами в дерматології, які потребують подальшого вивчення щодо безпеки та ефективності.
