ПРОЗАПАЛЬНІ ЦИТОКІНИ ЯК ДЕТЕРМІНАНТИ РІВНЯ ICAM-1 У ДІТЕЙ ІЗ COVID-19 ТА МУЛЬТИСИСТЕМНИМ ЗАПАЛЬНИМ СИНДРОМОМ

Анотація

Вступ. Молекула міжклітинної адгезії 1 (далі – ICAM-1) є трансмембранним глікопротеїном, який широко експресується різними типами клітин і відіграє важливу роль у перебігу інфекційного процесу та формуванні імунної відповіді на патогени. Експресія ICAM-1 підвищується за умов запалення та регулюється прозапальними цитокінами, зокрема IL-1β, IL-6 і TNF-α. Однак взаємозв’язки між цими цитокінами та ICAM-1 у дітей із SARS-CoV-2-асоційованими захворюваннями залишаються не досить вивченими. Мета дослідження – визначити ключові детермінанти рівня ICAM-1 серед прозапальних цитокінів IL-1β, IL-6 і TNF-α в дітей із COVID-19 та MIS-C з урахуванням віку та статі. Методи дослідження. Обстежено 285 дітей віком від 1 місяця до 17 років: 200 із COVID-19, 40 із MIS-C та 45 здорових дітей. Рівні ICAM-1, IL-1β, IL-6 і TNF-α визначали методом імуноферментного аналізу. Для статистичної обробки застосовували критерій Краскела – Волліса, коефіцієнт рангової кореляції Спірмена та множинну лінійну регресію. Результати й обговорення. Рівень ICAM-1 у дітей із SARS-CoV-2-асоційованими станами був статистично значуще вищим, ніж у контрольній групі, і становив 2,45 (1,91; 2,96) нг/мл у пацієнтів із COVID-19, 3,22 (2,57; 4,33) нг/мл у дітей із MIS-C та 2,05 (1,81; 2,28) нг/мл у групі контролю (H = 33,49; p < 0,001). У групі COVID-19 встановлено статистично значущий позитивний кореляційний зв’язок між рівнями ICAM-1 та IL-1β (rs = 0,23; p = 0,032). Водночас рівні IL-6 і TNF-α не мали статистично значущих кореляційних зв’язків з ICAM-1 (p > 0,05). За результатами множинного регресійного аналізу IL-1β виявляв незалежний позитивний зв’язок із рівнем ICAM-1: підвищення концентрації IL-1β супроводжувалося збільшенням рівня ICAM-1 незалежно від клінічної групи, віку та статі дитини. Висновки. У дітей із COVID-19 та MIS-C підвищення рівня ICAM-1 поєднувалося з активацією прозапальних цитокінів; IL-1β був незалежним чинником, асоційованим із рівнем ICAM-1.
