СУЧАСНІ СТАНДАРТИ ОЦІНКИ ФУНКЦІЇ НИРОК: ВПРОВАДЖЕННЯ РЕКОМЕНДАЦІЙ KDIGO ТА EFLM У ЛАБОРАТОРНУ ПРАКТИКУ

Анотація

Вступ. Хронічна хвороба нирок (далі – ХХН) посідає провідне місце у структурі глобальної смертності, що зумовлює потребу у превенції та стандартизації ранньої діагностики захворювання. Незважаючи на те, що лабораторні дослідження є ключовим інструментом оцінювання ниркової функції, у європейській лабораторній практиці досі спостерігається критична гетерогенність методів, розрахункових формул і одиниць вимірювання. Мета дослідження – обґрунтувати необхідність використання єдиного лабораторно-діагностичного підходу до ранньої діагностики ХХН на основі міжнародних рекомендацій Kidney Disease: Improving Global Outcomes (далі – KDIGO), EFLM і даних сучасної наукової літератури. Методи дослідження. Проведено детальний аналіз і систематизацію підходів щодо використання новітніх біомаркерів та інструментів оцінювання ХХН відповідно до міжнародних рекомендацій (KDIGO 2024), напрацювань комітету із хронічної хвороби нирок EFLM та релевантних джерел із наукометричних баз даних “PubMed”, “Scopus”. Пошук здійснювався за ключовими словами: “chronic kidney disease”, “estimated glomerular filtration rate (eGFR)”, “creatinine”, “cystatin C”, “albuminuria”, “CKD-EPI”, “EKFC”, “KFRE”. Результати й обговорення. Проаналізовано преаналітичні й аналітичні обмеження методу Яффе для визначення креатиніну та доведено перевагу ферментативних методів. Визначено, що впровадження формули CKD-EPI 2021 року в Європі призводить до систематичного штучного заниження швидкості клубочкової фільтрації (далі – ШКФ) та артефактного збільшення поширеності ХХН на 25 %. Обґрунтовано впровадження популяційно-специфічної формули EKFC для дітей від 2 років і дорослих, яка базується на статево-віковому індексі Q. Визначено правила розрахунку ШКФ за комбінованою формулою (креатинін/цистатин С) та стандартизацію оцінювання пошкодження ниркового фільтра через співвідношення альбумін/креатинін (ACR) у разовій ранковій порції сечі з обґрунтуванням відмови від застарілого терміна «мікроальбумінурія». Окреслено роль прогностичного рівняння KFRE для оцінювання ризику ниркової недостатності. Висновки. Раннє виявлення ХХН залежить від коректного впровадження новітніх біомаркерів, способів оцінювання швидкості клубочкової фільтрації та гармонізації показників лабораторної звітності. Віддання переваги ферментативним методам визначення креатиніну, імплементація цистатину С та показника ACR у щоденну практику, а також використання релевантних рівнів для оцінки рШКФ дозволить уніфікувати діагностичний процес і оптимізувати оцінювання ризику ниркової недостатності в пацієнтів.
