Анотація

Актуальність. Обмежена здатність нейронів до самовідновлення робить патології нервової системи однією з найважливіших проблем сучасної медицини, де традиційна терапія переважно впливає на симптоми, але не відновлює втрачені функції ЦНС. Застосування мезенхімальних стовбурових клітин (МСК) відкриває нові підходи до лікування захворювань нервової системи, проте потребує систематизації механізмів їх впливу на регенеративні процеси у нервовій тканині. Методи дослідження. Проведено систематичний аналіз наукових публікацій 2012–2026 років із баз даних PubMed, Scopus, Web of Science. До аналізу включали систематичні огляди, клінічні та експериментальні дослідження, що висвітлюють механізми клітинної терапії неврологічних захворювань. Мета дослідження. Оцінити та систематизувати сучасні наукові дані щодо молекулярних механізмів впливу мезенхімальних стовбурових клітин при захворюваннях нервової системи, а також виділити ключові біомаркери, що застосовуються для оцінки ефективності клітинної терапії неврологічних захворювань. Результати. Встановлено, що терапевтичний ефект МСК реалізується паракринною секрецією екзосом, факторів росту (BDNF, GDNF) та протизапальних молекул. Мезенхімальні стовбурові клітини інгібують прозапальні каскади (NF-κB/NLRP3), стимулюють проростання аксонів і синаптогенез (через каскад PI3K/Akt). Серед біомаркерів ефективності клітинної терапії при захворюваннях ЦНС виділено легкі ланцюги нейрофіламенту (NfL), гліальний фібрилярний кислий протеїн (GFAP), основний протеїн мієліну (BMP), прозапальні цитокіни (TNF-α, IL-1β). Висновки. Терапія стовбуровими клітинами та їх екзосомами є перспективним інструментом імуномодуляції та нейрозахисту, проте ефективність її клінічного застосування залежить від оптимального терапевтичного підходу, своєчасності втручання та контролю динаміки об’єктивних біомаркерів.
