In a number of previous articles, we dwelt on the special importance of trigger processes in biology and medicine [1-3]. The importance of trigger effects in apoptosis has also been emphasized [2, 3]. The latest research allows us to consider systemic biological processes from another side. Spatial trigger waves are a biological phenomenon that explains the inclusion and transmission of information quickly, reliably, and at significant distances.</p> <p>Results. The role of spatial trigger waves in the processes of apoptosis, anastasis, the development of inflammation, as well as the recurrence of an acute inflammatory reaction and other events is considered. It was postulated that the main features of the NF-kB regulatory network are the spatial combination of the functioning of fast positive feedback loops and slow negative feedback, which is necessary for active propagation of cytokine waves. It has also been postulated that NF-kB is involved in the mechanism responsible for the propagation of trigger waves and for fairly reliable signal generation, providing short exposures of the toxic effect of the produced cytokines.</p> <p>Conclusions. The available data suggest that NF-kB is a system component involved in the mechanism of generation of chemoattractant propagation waves — cytokines. It has been suggested that anastasis is associated with the spread of trigger concave spiral waves, which carry energy from the periphery to the center of the processes of reversing apoptosis.</p> O. System research in medical science and education is often based on data with a large number of attributes. Therefore, the problem of reducing the dimension of the data is actual, while keeping data with as much variation as possible. At the same time, such problems can be considered as problems of machine learning, which can be solved using artificial intelligence algorithms (for example, artificial neural networks.) The interpretation of the results of the application of classification algorithms without prior reduction of the dimension of such tasks is a difficulty for the decision maker. The results may be simplified by the reduction of the dimension of the task.</p> <p>Materials and methods. The method proposed and applied in this paper is described in terms of machine learning. Namely, the mathematical description of the tasks of machine learning in research in the field of medical education is based on the following data. We have a set consisting of tuples. Depending on the problem under consideration, certain sets of these tuples will be used for training, testing and prediction. An arbitrary tuple consists of input data (we will call them according to the established terminology of machine learning as attributes) and output data, which are attributes of the class. As a result of reducing the dimension, we obtain a certain numerical matrix, where these data can be used in variety of problems of machine learning. In order to study the possibilities of reducing the dimension of the medical education assessment tasks, the results of the medical students at TSMU were considered (the volume of the general population was 248).</p> <p>Results. In the work an approach which is based on application of the method PCA with the purpose of determining principle reasons (current and final progresses), which influence on the results of license integrated exam, is offered.</p> <p>Using the visualization, approach allows us to represent data on a plane in the coordinates of the first two main components. This allows us to identify the main changes in the attributes that cause the data to belong to certain groups. So, using such a visualization to the student data, we can get graphic representations of the «main» performance indicators (current and final) that predict the passing the licensed integrated exam «Step 1».</p> <p>Conclusions. Reduction of dimension is a very important element in the analysis of statistical data in medical education. Due to reducing the dimensions, we achieve not only that we simply need to analyze less data. The most important is that after the reduction, they often show us more information than before reducing the dimension. Many dependencies in medical education become more readable. It often happens that the amount of data we have is beyond the scope of our perception and then it is very easy to get lost in their analysis.</p> V. P. Martseniuk, Yu. V. Dronyak, I. V. An attempt was made to link the basic conceptual approaches of systems biology: networking, modular thinking, emergence, as well as biological interaction limits and robustness with clinical medicine. With the help of these conceptual approaches, the growth of robustness (biological stability) and stability in the development of cardiac diseases has been explained.</p> <p>Results. Possible pathways from modular activation to clinical phenotype are analyzed. It is postulated that systemic medical and systemic-biological studies are crucial for ensuring the success of efforts in the early diagnosis and personalized treatment of patients with cardiovascular pathology. They must prioritize and lead to the achievement of specific and realistic goals for strategies to intervene in the prevention of cardiovascular diseases. It is also shown that system-medical presentations provide a deeper understanding of which risk factors require the most attention, which drug-based approaches will be most effective and feasible in the context of limited time and cost resources.</p> <p>Conclusions. Finally, it is concluded that the development of agreed key indicators is an important next step in system research. Alignment of key indicators will ensure effective monitoring of the course of coronary heart disease and an assessment of the dynamics of risk factors.</p> O. P. Mintser, V. M. Surgical treatment of patients with primary and postoperative ventral hernia remains one of the important problems in the surgery of abdominal cavity. About 60 % of patients are of working age. The results of surgical treatment of postoperative ventral hernia indicate a significant percentage of recurrence, which is 4.3-46 %. This problem is relevant especially in cases where the inappropriate method of alloplasty and mesh implant is selected and in the early postoperative period a number of general complications arise and lead to recurrence. The latter require a detailed analysis of the reasons for their occurrence and the development of individualized prescriptions before choosing the method of surgery.</p> <p>Modern information technology greatly facilitates solving prediction problems. Currently, neural networks are widely used for early diagnosis.</p> <p>Materials and methods. Between 2001 and 2017, an in-depth comprehensive clinical-instrumental and laboratory examination of 1419 patients with primary ventral hernia (PVH) and postoperative ventral hernia (POVH) was carried out, with further evaluation of the nature of complications in the early and late postoperative period. For more in-depth analysis and clustering of the surveyed under study, a neural network approach was used with the help of NeuroXL Classifier add-in for Microsoft Excel.</p> <p>Results. In order to determine the value of the combination of changes in certain parameters for the prediction of the development of relapses of ventral hernias, a neural network clusterization of the results of the patient's examination based on the parameters of relapse, age, gender, blood group, concomitant pathology, etc. was performed. For the algorithm of neural network clustering the parameters proposed by the same program were selected. The largest number of patients with the first relapse was referred to the cluster 1, which was 8.4 %, where the highest number of patients with heart failure, chronic obstructive pulmonary disease and diabetes was noted. Most patients with relapses were found in the cluster 4, which was 27.6 %. The most commonly encountered patients with undifferentiated connective tissue dysplasia and the second blood group were also included in this cluster. At the same time, the average age in this group was the lowest and amounted 49.4 years. Analyzing the results of clustering of patients with relapses of ventral hernias by types of surgical interventions, it was determined that this cluster had the highest incidence of patients with primary ventral hernias, who underwent an onlay type surgery using lightweight mesh. The highest number of relapses (R1-R4) fell on the cluster 2 (19.1 %), which showed the highest number of women compared to the clusters 1 and 3-5.</p> <p>Conclusions. Analysis of cluster portraits revealed that the risk group for relapse included of women with the first-degree obesity, as well as patients with heart failure, chronic obstructive pulmonary disease and diabetes. The presence of the second blood group, the relatively younger age (&lt; 49.4 years), and the diagnosed syndrome of undifferentiated connective tissue dysplasia gives a reason to predict a higher risk of relapse after ventral hernia recurrences in operated patients.</p> V. Desktop computer program designed to optimize the process of patient registration.</p> <p>Conclusions. The trends of technologies development of postgraduate training of doctors as well as their continuous professional development are considered.</p> <p>Results. It is emphasized that universities are still the ideal place for the transfer and production of knowledge. However, it is completely useless to compete with online education for universities offering a typical contact education. It is postulated that many technologies of transfer of knowledge practically exclude the solution of the task of standardizing trajectories of learning and ensuring an efficient, individualized transfer of knowledge.</p> <p>Conclusions. The opinion is expressed on the necessity of developing algorithms for the timely identification of individual characteristics in learning the teaching material and on the basis of their creation of adaptive teaching systems.</p> O. Since XXI century is the era of electronics and the rapid development of physics and mathematics, scientific and technological progress, then the algorithms of clinical examination of patients should be reviewed in accordance with modern scientific achievements and technical capabilities. The authors raise the problem of the need for a paradigmatic transformation of a number of issues of medicine in accordance with modern discoveries of physics and biology of the human body. The data of the history of scientific development of bioenergetics scientific direction are given in the article. The essence of the publication: This is to demonstrate the feasibility of the scientific and technical possibility of applying a number of innovative methods to computerized modernization of the diagnostic search for a therapeutic profile doctor at the present stage in order to increase the objectification and quality of the examination of patients in the clinic of internal diseases.</p> <p>Materials and methods. The analysis of literature data was carried out in the course of conducting a search study of methods for express assessment of valeological status. This is a fragment of the initiative research work «Development of algorithms and technology for introducing a healthy lifestyle in patients with NCDs based on the study of psycho-emotional status" (State registration No. 0116U007798).</p> <p>Results. It was noted the presence of a significant reservoir of new scientific knowledge from the field of biophysics and biology, which deepen the fundamental knowledge about the organization and functioning of the human body and confirm the presence of the bioelectromagnetic component in the human body during the literature review.</p> <p>Conclusions. The basis of scientific knowledge is available for the need to change the paradigm approach to the assessment of the bio-electromagnetic component of the human body in modern medicine. It requires further integration into classical medicine and a revision of the paradigm in the pathogenesis and diagnosis of NCDs.
